It´s usually a combination of a bad handbrake (only locks one wheel) and the driver turning excessively to compensate for it, resulting in understeer.

Reminds me of when I was teaching a friend of mine how to do handbrake turns, I got him doing a few good ones when he turned around and said that he could do better with the footbrake. Needless to say he went straight on (Fortunately I was teaching him in a wide open space so he didn't hit anything).
