If I recall correctly, the "+1" list started when BP made the idea: "Or just make it so your trip isn't lost when your car breaks down", but actually we still lose our trips when our car breaks.

In real life do you get a countdown when your car is about to breakdown or is it more often spontaneous?

No, there is not a message saying "Your car is going to break down", but there are lights on the dashboard which light up when something is going wrong.

They doesn't necessarily mean in 5 miles you break down. It's an indication rather than an exact science or anything.
