Could you still update your original post using the correct format? Trust me, it's ideal to put as much effort as possible on an unban request. Otherwise, the admin dealing with it might get the idea that a user don't really care one bit about his/hers unban request. In that case, there's a very little chance the request will actually end up being successful. If there are some points you can't answer, for example if you haven't got a replay, just type in N/A. That's just an example.
