Ok a few things missing here.

1. The timestamp of the incident in the replay is missing, there is nothing at 14.55 on the replay.
2. There is no description of what has happened or what to look for.

Also you mixed up the lfs username with the gamename, it should look like this:

● Their LFS GameName(s):Warlord
