Afaik it does not spawn in online mode.

EDIT: Screw rockstar. They now removed the hidden packages, how the *Removed* are you supposed to make money once you beat the game. I'd like to buy more than 3 Z types before I'm out of money... @mylostCars: yes im glad it isnt just me and somehow this can be fixed. Spent over 100k on both the cars i lost and i dont want to lose any more.

The red smoke indicates an incoming crate drop which would have several rewards inside, namely money, snacks or RP. It just so happened that we spawned right where it would drop
